Information about LSTA grants and other funding opportunities.

When to Survey

Many LSTA-funded programs require a standard survey from IMLS. When we complete the State Program Report for all grants awarded within a grant year we include these surveys to provide proof of assessment. This is an important part of grant accountability and assessment. Was your program successful? How valuable was it to attendees?

If your library system has an LSTA-funded program for the general public or for library staff, we would like a survey conducted. If LSTA funds paid to send a library staff to a conference (other than MLA) we would like a survey conducted. You may tailor one of the IMLS surveys to meet your needs but the essential questions on each survey need to be asked, please. For the final grant report that is required for all LSTA grants, you may include your statistics as part of the final report.

Surveys to Complete

Every LSTA-funded grant or program needs to be evaluated and assessed. All of the linked surveys are required for the program below that corresponds. The Maryland State Library Agency will aggregate the anonymous data to deliver to the Institute of Museum and Library Services with the appropriate State Program Report.

Instructional Program for the General Public - Use this survey to evaluate LSTA-funded library programs that had participation in or attendance by the general public.

Instructional Program for Library Workforce - Use this survey to evaluate LSTA-funded training programs for library staff. (Any training led by a MSLA employee is LSTA-funded)

Questionnaire for Library Workforce about a Resource - Use this survey for library staff to evaluate a resource, such as any LSTA-funded staff handbooks, procedure manuals, and/or any items that benefit staff.

How To Report Survey Data

If a library system chooses not to use the survey links above, data must be delivered to the Maryland State Library Agency in an aggregated form. When reporting survey data for LSTA-funded program Final Reports, please combine surveys and report the following information:

  • # of surveys gathered
  • # of people who answered the 5 survey questions as:
  • Strongly Agree #
  • Agree #
  • Neither strongly agree or disagree #
  • Disagree #
  • Strongly Disagree #

Please do not provide percentages of survey data and/or raw data that has not been aggregated. Thank you so much for helping to assess programs to be good stewards of federal funds.
